I'd say ki sensing.
Ya can act tired, and fool people; bu my eneral assumption was that as someone becomes more tired, their energy drops. Obviusly, it's not perfect, especially whe dealin with guys like the ihters who can suppress their powers(and so, seem weaker or tired) so, in that case, probably a mix of the two, and the fighters own judement.

Well, I find that many combatants seemly seem to be hamming it up when it comes to chi-loss and stamina. #18 and Vegeta's fight comes to mind - one can appear to be as such but #18 just drew out the fight and appeared roughed up just to compound her superiority (as SSj Vegeta's chi dropped away fairly quickly, and that it went to the notice and observation of Piccolo vindicates this). As for sensing chi-signatures or just sensing any one amount of chi so as to be an indictator... remember that a warrior's chi can be suppressed or masked to the point where whether they're bleared or not would remain an enigma. However, in Goku's case, I'd say it's just done for plot devices (as maybe the combatant with superiority in power simply doesn't have anything to do without being purposeful or being at the behest of some incentive or thing) or to inanely draw out things... so when that kind of pain and stamina loss is being shown, it isn't for purpose, so much as it is for rejection and giving up on all accounts (since it does make Goku seem like a bit of a dunderhead).

I'd have to agree...your eyes can be fooled and so can ki sensing (power level suppressions) and scouters...fail. But out of those three I would say ki sensing is the -most- reliable.
