Now correct me if I'm wrong, but doesn't =/= mean equals? As in Pain=Damage, as in Pain means you have taken damage?

Though it's not really a legitimate sign, a lot of people tend to use =/= to represent non-equality, as in the two sides are not equal.
Damage can result in pain (although if the nerves are destroyed, there can be bodily damage without pain present), but pain doesn't necessarily result in damage. I could have a nerve temporarily pinched, which would cause pain well after the "pinch" subsides, which would result in there being pain, but no discernable sign of damage.

That is true, but in this instance, unless Tiens Kikoho is some kind of pain-ray(Which there is zero evidence to support in the slightest) it means that the fact that Cell felt pain means it did inflict damage.
