Let's see, going by your rules and balanced powers....

Luffy From One Piece would beat Itachi hand to hand, because to beat Luffy you need to be much stronger than he is since he's a rubber man. This means with balanced powers, it's like an automatic win for Luffy, although maybe using Kunai could help Itachi......

Sensui from Yu Yu Hakusho would beat Itachi hand to hand and balanced powers, because Sensui has the ability to beat down people that are stronger than him thanks to his Ryshuyuken (Butchered the spelling I bet lol) technique of blocking which is literally, the best possible form of martial arts in Yu Yu Hakusho. Sensui's ability to read his opponents is also very good too, he knew enough to turn his cheek while Yusuke punched him even after only one brief confrontation.

Itachi vs Cthulhu

Very obviously, Cthulhu wins for the following reasons:

-Huge body size(can stomp over Itachi at any given time)
-Tentacles(can seize Itachi, and then properly bash the sucker into a wall or something)
-Huge claws, to RIP AND TEAR Itachi.
-Cannot be actually killed - only returned into a death-like state of sleep.
-Supposedly even seeing Cthulhu drives any man mad - even easier for Great Cthulhu to kill Itachi.

Cthulhu is the winner.

Nami, Zoro, and Sanji have puched Luffy and "hurt" him and they're all arguably weaker than him. So, supposedly, Luffy can be hurt by blows powered by an emotion, and we all know that there is one emotion that Itachi excels in. Hatred.

If it's equal power, there aren't very many who could beat him due to the Sharingan, but they do exist. Mostly immortal brutes, but still.

But it's literally stated that you must be way stronger than him to hurt him.

Those are probably comical momments you're thinking of.

I have a few more:

Alucard from Hellsing is basically immortal and has a gun..... You get the picture.

Karasu from Yu Yu Hakusho would kill Itachi effortlessly since his bombs are impossible to even see without a certain amount of spirit/demon energy

Several characters from One piece like Enel, Ace, and smoker can't be beaten unless their natural elemental enemy hits them, or their powers are absorbed, so they technically can't lose to Itachi.

Dabura from DBZ can win simply by spitting on Itachi

Sephiroth
-Itachi Vs Sephiroth (Jenova) : Itachi dead...

powers
-Super human enhanced speed, and duration
-Has supreme mental powers beyond that of Itachi's (KO'd Itachi)
-Aerial abilites: teleportation, flight
-Black Materia: Summons a huge meteor, inflicting serve damage to the Solar System itself, if Jenova inlfuences Sephiroth, do so so it could wipe out the Earth.
-Jenova Cells:Oh yes Sephiroth is immortal, you can inflict damage upon his shell's mind or body, but in order to do that you must surpass Sephiroth's godly powers. Then he'll just incarnate himself again...

Kuwabara from Yu Yu Hakusho: One slice with the dimesnion sword and it's all over.

Chuck Norris: Come on, can anyone beat this guy besides Bruce Lee?

Steven Segal: Again, can anyone defeat this guy?

Most of the people you mentioned could only beat Itachi because they would use some sort of special ability, when this match-up clearly states that this is hand-to-hand only. Only good candidates so far are Luffy, Buu, or those random guys you mentioned who "can't be beaten unless their natural elemental enemy hits them". The rest need some sort of special skill to deal with him, and you are all ignoring Itachi's Sharingan, that gives him a massive edge when dealing with different fighting styles, since he can instantly copy them.

And Shirogetsu, apology not accepted, plz go die =/. Nah, j/k, you are forgiven.

Itachi could perceive his movement with the Sharingan and evade it in time, like, with ease.

He said weapons were allowed.... So maybe Karasu's bombs are a bit out of the question.....

Dabura is a sure win, and I guess with Alucard it sort of Depends how Itachi can handle going against a gun, considering they have no guns in Naruto.....

Got it Vamp out of Metal Gear Solid 2 (Sons of Liberty), he only died by drowing in some freaky water (but he even survived being drowned he appears in Metal Gear Solid 3), and he got shot point blank in the head as well as in the neck and came back twice lived...His basically a wierd vampire (wihout any of the weaknesses), he can walk on walls and water too. Vamp basically an immortal killing machine.
Now he can also, can perceive a persons movements with his eyes (ability to read a man's every muscle movement before he makes it ability to read a man's every muscle movement before he makes it - Sharingarn), he has decent hand to hand combat skills and he might uses a hunting knife. The Romanian Vampire would present himself to be an interesing challenge for the great Uchiha Itachi.
