I haven't actually watched the movie dubbed. I said earlier that I detest live action dubs. I never said the heart attacks ruined the movie, I said the shitty acting did. The heart attacks are a fine example of that.

It's kind of like the manga ending, except they replaced Near with L. L replaces Misa's Death Note with a fake (like Near did to Mikami) and writes his own name in it, setting his time of death for 23 days later (a rule states, if you recall, that a person can only live for up to 23 days after having their name written in the notebook; the "23-day rule").
Therefore, when Rem writes Warari and L's names into her Death Note, Watari dies, but L already has a time of death set in the other notebook, so he fakes his death. Light thinks he's the shit and that he has won. Later, Light writes his father's name into Misa's (fake) notebook, writing that he will bring the other notebook (the one they took from Takada, who takes the Yotsuba Kira's place in this movie) from the safe and give it to Light before dying. Misa doesn't understand how Light could kill his own father.
L, who is spying on Light and Misa with hidden cameras, has Light's dad meet Light with a suitcase, which turns out to be empty. Light is freaking out, wondering why the Death Note didn't affect his father like he had wrote. L then pops up and explains everything I just wrote above (the audience didn't know he was still alive until this point). Light then tries to write his father's name (?!?!) on the paper in his watch, but is shot by Matsuda.
Then, the manga ending plays out, with Light begging Ryuk for help and getting killed. Light dies in his father's arms, saying that it was all for a better world and whatnot. Ryuk asks if L's going to use the notebook now, but he says he won't; he's going to burn it. Ryuk flies off, thinking L's boring. 23 days later, he dies eating a bar of chocolate, next to a picture of Watari.
A year later, Light's family visits Light's grave; his mom and sister think he died fighting Kira. Misa is shown in her room, which has been made into a shrine of Light. She thinks that somehow, something importing in her memories is missing. Ryuk is then shown flying around Tokyo Tower, and he laughs.
Cue the Red Hot Chili Peppers.
(Note that the new Death Note movie, L: Change the WorLd, takes place during the last 23 days of his life)
