Well Goku did say it took him alot of practice to learn Instant Transmission AND to master the concept of going Super Saiyan at will. So if someones training hard they'll odviously get a massive power up. Plus Goku could have asked Bulma's Dad for a gravity machine if his normal training with Piccolo & Gohan wasn't working.

I subscribe to the theory of depreciating gains. When you're that strong, training, unless in extraordinary conditions, shouldn't increase your power too greatly.
For instance, an Olympic level runner may train hard every day for the year coming up to the Olympics, but it's not going to double his speed. It'll maintain his speed, perhaps a small edge. Now, in DBZ, it's somewhat different, because the levels they reach already shoot far past the average, so the depreciation of power gains wouldn't be as extreme, but I definitely think it's there.
